A Light at the End of a Tunnel

As a high-risk maternity centre, Royal Columbian provides comprehensive services for both infants, children and their families. As a referral centre for high-risk maternity, RCH delivers 3,000 babies annually – and with the Fraser Health region’s projected population growth, this number is sure to climb. These include pregnancies with co-existing health conditions, those at risk of delivering prematurely, and those diagnosed with severe obstetric complications—like Makayla, whose son, Marcus, was born at 24 weeks and four days, weighing just 755 grams.

Makayla recalls being sick throughout the whole pregnancy. She was in the hospital for a week because she couldn’t keep anything down, and had to be given an IV for nutrition and fluids. Halfway through the second trimester, Makayla underwent a cervical cerclage, a procedure meant to reinforce the cervix that has the potential to open early. Later on, her water broke, and after confirmation, Makayla was on her way to RCH to deliver.

Makayla wasn’t expecting to hear her baby cry once he was born in early January 2021, but when he did, it made his mother so happy—a happiness she still feels to this day.

Makayla received follow-up care while Marcus was intubated and taken to the neonatal intensive care unit, with dad close behind him. The family’s life revolved around the NICU for the next 132 days, going through all kinds of ups and downs and growing an appreciation for the hospital’s neonatal team. The parents didn’t know if he was going to make it or not, and during the first few days, Marcus had several blood transfusions and needed antibiotics due to bleeding around the left side of his brain.

Ten days later, while he was still connected to the ventilator, Makayla was able to hold Marcus for the first time. And a few weeks later, Makayla was happily surprised to see her son off the ventilator and breathing with the help of a less invasive form of breathing support.

Marcus was moved to a crib and was big enough to wear clothes nine weeks after his birth. Marcus was getting stronger and required less medical supports, slowly but surely.

On Vancouver Island, in May, Makayla’s grandfather sadly passed away, but by this point she was comfortable leaving her son in the care of his medical team so she could go and say goodbye. The nurses sent Makayla pictures of Marcus every day.

After a long journey, Marcus was finally able to go home ten days before the start of summer. Five months later, and Makayla has said that her son is doing great. While still having a G-tube, he is eating everything by mouth and is hitting all his milestones.

“Some days you feel like you’re going to be there forever, and it’s never going to end,” she says. “But, there is a light at the end of the tunnel. I think that was the hardest part for me. I hope sharing my story will help other parents see what the journey is about.”

Mental Health is more important now, than ever!

Feelings of anxiety are common with the stresses of everyday life, and the challenges of the past two years have put additional strain on many who are already feeling overwhelmed. It is no wonder that one-in-five British Columbians are affected by mental health challenges or illness in any given year. There are times when we need more support than our friends, family, and community can provide. Grateful patient Miranda Tymoschuk knows first-hand that Royal Columbian Hospital is a place for that support.

Miranda was born at Royal Columbian Hospital in 1996 and diagnosed with a rare condition in her leg which caused it to grow abnormally. The Tymoschuks were told that Miranda would have to undergo a series of surgeries to lengthen and straighten the bones in her leg. Then, at 5-years old, her dad passed away suddenly from a heart attack. With little family to rely on, Miranda’s mom was left alone to care for three young children, including Miranda with her significant health challenges.

Did you know up to 80% of ill or injured children experience traumatic stress reactions following a life-threatening illness, injury, or painful medical procedure?

Miranda went through her first surgeries when she was eight-years-old, and the second set when she was 12. After, she was eager to be a “normal” kid so she tried to ignore the abdominal pain and other symptoms that developed. As it worsened, Miranda saw many doctors and after years was finally diagnosed with Crohn’s disease, with additional complications. Just before Christmas Eve 2012 she became extremely unwell and was admitted due to a serious infection originating in the gallbladder. Sick and in unbearable pain, it was a very scary and lonely Christmas for the Tymoschuks.

What was most challenging, however, were the nightmares, flashbacks, anxiety, and depression that consumed Miranda after being discharged from the hospital. There were moments where she doubted whether it was worth it to keep going. With the chronic pain entangled with emotional pain, it wasn’t long before Miranda became addicted to opiates.

“I was ashamed to be struggling and it took time to ask for help. I was finally diagnosed with PTSD, connected with a great psychiatrist, and started counselling.”

Every day, people all around us – our friends, family members, and colleagues – experience struggles we may not even realize. But you can help Royal Columbian Hospital meet the pressing needs of our communities.   

Royal Columbian Hospital Foundation is incredibly grateful for the support for Royal Columbian’s Mental Health and Substance Use Wellness Centre, which opened in 2020. The centre has doubled the hospital’s capacity to care for patients in need of mental health care. Featuring increased patient privacy, access to green space, as well as room for exercise and creativity, the centre has been designed to facilitate recovery.

With the help of the Royal Columbian team, Miranda have gotten her life back. She is now in university and hopes to become a doctor; Miranda’s dream is to help others the way Royal Columbian doctors helped her. She currently works as a peer counselor for youth and fundraises for Royal Columbian and other organizations.

With almost double the current footprint, redevelopment will significantly increase the current capacity of RCH by over approximately 50% to 675 beds and will provide highly sophisticated critical care technologies and treatments. Coupled with the Mental Health and Substance Use Wellness Centre that opened in the summer of 2020 as part of the first phase of redevelopment, the Acute Care Tower will allow our physicians and health care team to treat 11,300 more patients each year.

Heart of gold

Generosity has the power to create a ripple effect that can last for generations. As evidence, look no further than a thoughtful philanthropic gift made decades ago that set the course for the growth of cardiac care at Royal Columbian Hospital. Over the past 50 years, heart services at the hospital have expanded to the point that Royal Columbian is now the busiest cardiac centre in BC, and it has become the busiest in Canada for a key, life-saving heart attack treatment.

Royal Columbian’s first heart catheterization took place more than 50 years ago, something that was made possible in 1969 following a $100,000 donation from the P.A. Woodward Foundation.

At the time, it was the largest private donation ever made to the hospital, and it triggered a successful fundraising campaign to match the contribution.

The equipment that was purchased allowed for cardiac imaging to be done for the first time at Royal Columbian. The first heart catheterization was performed in November 1969.

Today, the New Westminster-based hospital provides emergency cardiac care for a third of British Columbians who live, work, or play within the boundaries of the large Fraser Health Authority. In addition to the most open-heart surgeries in the province, Royal Columbian performs more angioplasties than any other hospital in the country to restore blood flow to people who have suffered heart attacks because of clogged arteries.

Cardiac services are again set for tremendous growth and advancement as Royal Columbian undergoes phase 2 of its unprecedented $1.49 billion redevelopment. A new Acute Care Tower, set to be complete in 2025, will include increased care for heart disease and capacity to treat more patients.

Revolutionizing Patient Care

The ‘crown jewel’ of Royal Columbian Hospital’s $1.49 billion redevelopment – a new Acute Care Tower – is increasingly taking shape since construction started in 2021.

The vision is to create a world-class hospital: a state-of-the art facility that provides uncompromising care and remarkable patient experiences. With a larger footprint and the introduction of cutting-edge technology, Royal Columbian will continue to respond to the needs of patients in their most urgent, life-altering moments.

A transformation in critical care

Royal Columbian’s redevelopment will create a 60-bed Critical Care Unit on the fourth floor of the new Acute Care Tower to increase capacity. This unit will serve all of Fraser Health and at the moment includes 28 Intensive Care Unit beds, 17 High Acuity Unit beds, and 15 Cardiac Surgery Intensive Care Unit beds. Additionally, the fourth floor will include a 17-bed Cardiac Intensive Care Unit, which will care for critically ill non-surgical cardiac patients, and seven additional medical surgical beds.

A transformation in emergency and trauma care

The new Acute Care Tower will house a larger Emergency department with 75 state-of-the-art treatment bays – increasing total space by more than half. It will include four trauma bays and a new Imaging Unit, strategically placed within the Emergency Department to drastically speed up access to diagnostic procedures. Also, one operating room will be purpose-built and always available for trauma patients, to help avoid interrupting scheduled surgeries.

A transformation in interventional care

The creation of an interventional super floor in the Acute Care Tower with multiple interventional suites will equip the hospital with advanced, state-of-the art imaging technology that allows for absolute precision. This floor will house eight interventional suites that will be universal, meaning that if needed, any suite could be used for either interventional cardiology or radiology. At opening, three suites will be used for interventional cardiac procedures and five suites for interventional radiology.

A transformation in care for families

With an entire floor devoted to both maternity and neonatal intensive care, along with a new pediatric department, there will be a significant emphasis on family-centered care in the Acute Care Tower.

It will house a new 48-bed Maternity department, with most rooms slated to be single-patient rooms to allow families the opportunity to bond in a private and dignified setting. The new NICU will feature 24 single-patient rooms designed to support babies who require specialized one-to-one care. The new pediatric unit will provide every child and family with their own private room with an ensuite washroom and a sleeping area. These changes are not only designed to increase capacity and efficiency; they will also allow for increased privacy, patient dignity, and family bonding.

A transformation in surgical care

Royal Columbian’s surgical program will benefit from the addition of eight operating rooms in the Acute Care Tower, with three more outfitted for future use. Three ‘hybrid’ operating rooms of more than 860 net square feet will be equipped with state-of-the-art imaging technologies — allowing surgeons to use real-time imaging to diagnose, treat, and evaluate patients quickly, improving the speed and quality of care for complex cases and trauma patients.  In addition, three cardiac ORs will open as part of Royal Columbian’s role as the health region’s emergency room for the heart.

Trauma physicians refer to the first hour after a traumatic injury as the ‘golden hour’ – they are the critical minutes for medical and surgical intervention.   When a traumatic incident occurs, the clock starts to tick. Every moment counts – and life hangs in the balance.

The Emergency Department, one of the most highly utilized in the province, is the entry point to trauma care, and both services will receive significant additions during the hospital’s redevelopment. The new Acute Care Tower will house a larger Emergency department with 75 state-of-the-art treatment bays – increasing total space by more than half.

As one of two Level 1 Trauma Centres for adults in BC, Royal Columbian is equipped, trained and certified to deal with large-scale catastrophes. It receives more Air Ambulance patients directly from accident scenes than any other hospital in the province.

In the new Acute Care Tower, the Emergency Department will include four trauma bays and a new Imaging Unit, strategically placed within the Emergency Department to drastically speed up access to diagnostic procedures.

Also, one operating room will be purpose-built and always available for trauma patients, to help avoid interrupting scheduled surgeries.

These additions and improvements will speed up admission, assessment and treatment. The hospital will have more space to accommodate all types of ER patients, and trauma patients can avoid being moved from one part of the hospital to another to get the scans they need.

As well as supporting emergency and trauma care, Royal Columbian’s redevelopment will include an interventional super floor with operating rooms and interventional radiology and cardiology suites, new and larger maternity, pediatric, and neonatal intensive care units, and multiple floors for acute and critical care patients.

Ushma wants to tell you about her colleagues, the ones on a medical unit at Royal Columbian Hospital who have looked after hundreds of COVID-19 patients since the start of the pandemic. We want you to know about the care and passion that goes into helping these patients, especially in these trying times. And we want you to know how much we appreciate the donor support and encouragement these last two years.

As a patient care coordinator on Royal Columbian’s COVID-19 unit, Ushma feels blessed to be part of a team like this, “My colleagues are so dedicated, so passionate, and so flexible with the amount of change that has occurred.

“I will never forget the first patient who was admitted to the unit because of COVID-19 at the start of the pandemic in 2020. The patient became quite sick, and we were working through our own stress and anxiety, each of us navigating through a lot of unknown to ensure we could look after this person to the best of our abilities.

“Since the first COVID-19 patient, we have cared for hundreds of others. We have celebrated those who recovered from their illness and returned home, and we have mourned for those who did not survive despite exceptional efforts. Throughout, we have supported each other and helped the team overcome challenging moments.” says Ushma.

The pandemic has evolved regularly during its five waves. The demands on healthcare have been constant, and our caregivers are fatigued. Serving a population of nearly two million, Royal Columbian Hospital has been at the centre of the battle. The support Royal Columbian Hospital Foundation receives through the lottery will help to continue to meet the region’s needs, now and into the future.
